Word from the President…

Having recently been a patient in the healthcare system, I find I must comment on the pain management I received in the different settings.  I had a total knee replacement for osteoarthritis.  I have OA in numerous joints other than the knee that I had operated.

The first setting was inpatient.  My post-op care varied very much by the nurse and shift.  As always nights were staffed the least and on surgery the workload was not particularly lighter on any one shift.  I found that knowledge of the “system” can be both good and bad.  I knew when shifts changed so I could try to time my pain requests at times when shifts were not changing if at all possible and to try to call right before they would be passing medications so they would not be making an extra trip.  The inpatient setting did assess pain regularly for the most part, but not 100% of the time nor all of the staff.  Still some room for improvement.

I don’t know if it was because I am a nurse and the staff knew this, but they freely talked in front of me about staffing issues.  Something I was taught not to do.  They also made comments about other nurses and their good and bad points.

I went from inpatient to an extended care rehab setting.  I was pleasantly surprised by the attentiveness of the staff to pain issues in this setting.  Again there was variance nurse to nurse and it was not always assessed, but for the most part requests for pain medication were promptly responded to.  Physical Therapy was very attentive to pain and planned therapy around pain medication.  Physical therapy also used several modalities to relieve pain.  They used a TEN’s, ultrasonic heat, positioning and ice.

Lastly I had home physical therapy.  The therapist always planned when the visit would be so I could take my pain medicine prior to the visit so I could work to my maximum during therapy.  They encouraged me to ice my knee after exercise to help with pain.  They never failed to assess the level of pain at a visit.

I certainly feel that we have made inroads into improving pain management across the various settings.  I was glad to get home where I was in total control of my pain management.  I have to say there are still some staff with “attitudes” that show when it comes to who is the authority about the patient with pain.  That only proves that change is hard, takes time and persistence.  We can not rest on our “laurels”, but must continually keep pain management on the front burner.  To do this means always being aware that pain needs to be assessed continually and reassessed.  Secondly, we always need to continue to educate ourselves, other healthcare professionals, and the community about pain and its management.

…In the News…

Unfortunately, BJH decided to eliminate Pain Integrated Services. This is not only unfortunate for the nurses who worked in this area, but is unfortunate for the patients in that hospital whose pain management will be less than optimal.

 

Also unfortunately, our cookbook did not have enough member participation to gather 300 recipes in time for a December publication. Hopefully we will be able to gather enough recipes by mid-summer to be able to offer a second cookbook for sale mid-November. In addition:  Karen presented improving understanding of diverse massage techniques to the Missouri Assisted Living Association. At this meeting she also had an opportunity to lead 4 round table discussions ..including one on aroma therapy. GO KAREN!

Meeting Minutes for September 25, 2008

MEMBER S PRESENT: Donna Baudendistal, Jean Brontoli,Patty Burhanna,  Cathy Flasar, Jo Hawkins, Gail Hurt, Dorothy Stratmen-Lucey

 

CALL TO ORDER: The meeting was called to order at 6 P.M. by chapter president Jo Hawkins.

 

MINUTES: The minutes of May 16, 2008 were approved as written.

OLD BUSINESS:

TAN TAR A

Dorothy Stratmen-Lucey discussed the need for more chapter members to volunteer for  work on  the TAN TAR A annual workshop. In previous years both the St. Louis and Kansas City chapters provided volunteers to work at the actual event . This past year Dorothy Stratman-Lucey did most of the preparation work and the St. Louis chapter members were not available to  serve as registration volunteers. Anyone wishing to volunteer for next years workshop should contact Dorothy as soon as possible.

The members discussed possible topics for next year’s workshop. Some mentioned were : Institutional Culture changes, Chronic Pelvic Pain in women, Chi massage, Acupuncture and other chiropractic techniques for pain, Healing Touch, Talking to Your Chronic Pain Patient, Case Management for  the Patient in Pain.

Some suggested speakers were Rebecca Ringo (talking to the chronic pain patient); Pat Dulle and Diane Pryor (healing touch), Dennis Baker (acupuncture) – Jean Brontoli knows him and will make contact with him, she also knows a chiropractor fro m Springfield who would be a good speaker- she will contact both as soon as we have an established date for the workshop. Dorothy will contact the Kansas City Chapter to get the date set.

MEMBERSHIP: We have a continued need for new members, especially as long term members retire from the profession. Joan Wentz distributed names for each member present to contact to  encourage current members to be more involved. Any ideas for increasing membership – contact Jo Hawkins.

 

FUNDRAISING:

Madeline Tucker needs more recipes if we are to do a second printing of our cookbook. The first cookbook was very successful.  If you have a recipe sends it to Joan Wentz or Madeline Tucker as a WORD document attached to your email. Please send by October 30, 2008.

 

NEW BUSINESS

OFFICERS:

            Jean Brontoli will assume the position of Chapter President as of January 9, 2009. The nominating committee – Gail Hurt and Donna Baudendistel will prepare a ballot for the December  meeting.  Officers to be elected are for Vice President, Secretary and Treasurer.

COLLEGE COURSE:

The members present discussed offering a 16 week course each semester for college credit through the St. Louis Community College.  Dorothy Stratman-Lucey will contact the college to discuss this. Members will serve as speakers as they have in the past for a similar continuing education class and as they are doing currently for a Pain Resource Nurse Workshop.

FUTURE PROGRAMS FOR CHAPTER MEETINGS:

Donna Baudendistal will contact the rep for Naltraxine (Relistor) for a March program.

Summary of Pain Resource Nurse Workshop of September 25 and 26, 2008

 

            A Pain Resource Nurse Intensive Workshop, held September 25th and 26th, 2008 was a resounding success, even better than anticipated.  This workshop was sponsored by SIU-E, and the Research Consortium Group of Southern Illinois. Val Yancey and Cathy Flasar, chapter members, are both on the research committee and Dr. Val Yancey is a professor of nursing at Southern Illinois University at Edwardsville. Cathy Flasar is a Doctor of Nursing Practice candidate working with Dr. Yancey.  The expert speakers were all members of our  local chapter, the St. Louis Regional Chapter of ASPMN.

When asked a few months ago, chapter members volunteered generously with their time and expertise. This made it possible to have both an interesting variety of speakers and presenters  who are experts in their area of pain management. The members who spoke at this PRN workshop were Jean Brontoli, Barb Chappuis, Cathy Flasar, Anne Hill, Gail Hurt, Diane Pryor, Joan Wentz and Val Yancey. Having all these members agree to speak provided many of expert experiences and opinions as well as all the basic information needed by these new Pain Resource Nurses.  Each speaker provided case studies for the attendees to delve into.  Diane even provided a group practice session in Healing Touch, which received very high ratings in the evaluations.

Twenty five nurses interested in becoming Pain Resource Nurses at local hospitals in Illinois attended both days. They demonstrated enthusiasm for learning and for discussing many issues and had many questions for the speakers. The attendees wrote excellent evaluations of the workshop and all the speakers. Many of them asked for follow-up workshops. Each attendee will be doing a project related to pain management as part of their commitment to becoming a PRN Nurse. The attendees were also encouraged to join the St. Louis Regional Chapter of ASPMN.
