A Word from the President…..
In the last issue of this
newsletter I mentioned one of the goals we need to review was becoming a more visible
presence in the community in order to let people know who we are and how we may
be able to help them. I made several suggestions and a brief discussion of this
goal was started at our March 3, 2005 meeting.
Have you given it hours of deep
thought? If so you are probably a recent retiree or a recent lottery
winner. I do hope you have or will give
this subject a few ponderous minutes. I (and the other chapter members who
attended) brought exciting news on this topic back from the recent ASPMN National
Conference.
In November 2003, the ASPMN-
Maryland chapter launched a POWER OVER PAIN Initiative. The developer of the
Power Over Pain Toolkit gave a report on the history of the development of the initiative
and their partnerships with the Maryland Pain Initiative and the American Pain
Foundation. This initiative is a vehicle for fighting the SILENT EPIDEMIC OF
UNDERTREATED PAIN, an epidemic we are all too familiar with. The premise of
this community initiative is that “the face of pain is hidden from the general
public eye” and that by holding meetings open to the community, people would
have the opportunity to share their pain experiences with others experiencing
pain or those sympathetic to their plight.
The goals of the Power - Over
- Pain initiative were to:
1) Promote a safe environment
for disclosure.
2) Give voice to the silent
pain public.
3) Clarify community-specific
barriers to access of pain management for a targeted population.
4) Allow for development of
solution-driven strategies that are focused, community-based and collaborative.
The ASPMN-Maryland chapter teamed up with
local churches and together with the ministers of the churches, chapter members
were able to help facilitate comforting and/or healing, as well as education on
community resources, through these meetings. They held eight meetings in 16
months. They were also able to identify and groom community-based advocates to
continue the process.
I will present more
information on the Power Over Pain Initiatives (other National Conference,
attendees feel free to jump in with comments during the meeting) and then open
it up to group discussion. The Power Over Pain Toolkit is free to any chapter
wanting to use it.

Palladone®
On September 24, 2004 the FDA approved a new extended-release formulation of the narcotic hydromorphone, called Palladone®. This is the first extended-release hydromorphone product (or Dilaudid®) available in the US.
Palladone® capsules utilize a controlled-release melt extrusion technology in which pellets of active drug within the capsule slowly release hydromorphone over a 24 hour period. This formulation is marketed in four strengths: 12mg, 16mg, 24mg, and 32mg capsules.
Palladone® capsules are indicated for the management of persistently moderate to severe pain in patients requiring around-the-clock analgesia with a high potency opioid for an extended period of time . Palladone® is not indicated for prn use or when therapy of a short duration is required. Furthermore, it should not be initiated in opioid-naïve patients. Palladone® is only indicated for opioid-tolerant patients who are taking at least: 60mg of oral morphine, or 30mg of oxycodone, or 8mg of oral hydromor-phone daily (or equianalgesic amounts of other opioids) for 7 days or longer. Palladone®16mg daily is equivalent to Dilaudid® 4mg 4 times daily. Overestimating the dose when initiating therapy can result in a significant overdose.
Palladone® should be swallowed whole. Alcoholic beverages or
medications (elixirs) should be avoided since they can degrade Palladone’s® controlled release matrix &
significantly increase the drug’s blood levels. Likewise, if the medication is
broken, chewed, dissolved, or crushed, the controlled-release properties of the
drug will be compromised and inappropriately high amounts of hydromorphone
would be rapidly absorbed by the patient with potentially severe or fatal drug
toxicity.
Palladone® undergoes biphasic absorption with a rapid initial peak concentration followed by a second broader peak with levels stable over 24 hours. Absorption is not significantly affected when administered with food. It has a mean elimination half-life of 18 hours. Metabolism occurs by direct conjugation in the liver. Patients with mild to moderate renal dysfunction have slightly elevated drug levels when compared to patients with normal renal function. Elderly and debilitated patients are generally more sensitive to the drug & require more conservative dosing. Dose determination should be based on standard equianalgesic dose table recommendations and modification of recommended dose based on patient characteristics (medical condition, past drug history, co-morbid conditions, concurrent meds, & pain characteristics). Patients initiated on Palladone® should have frequent assessments of their pain needs with dosage adjustment no more frequently than q2-3 days. Patients should receive constipation prophylaxis & an immediate release product for breakthrough pain.
Common side effects include: constipation, nausea, dizziness, somnolence, & urinary retention. An FDA-approved patient information sheet is required to be dispensed with each out-patient Palladone® prescription.

Pain
Management Certification Exam
Starting October 15, 2005 staff nurses will be able
to take the Pain Management Certification Exam, offered by the American Nurses
Credentialing Center (ANCC).
The
American Society for Pain Management Nursing (ASPMN) and ANCC collaborated over
the past several years in development of this certification exam. A dozen
nurses were recruited to write questions for the exam. There are 175 questions.
Of these, 150 are scored questions and 25 are nonscored pretest questions.
Pretest questions are for data collection only and cannot be distinguished from
scored questions.
Categories
of questions are: Pathophysiology (18), Pain assessment (43), Interventions
(24), Side effects (17), Patient/Family Education and Counseling (17), and
Collaborative/Institutional Issues (31).
Questions test pain concepts across the life span and in specialty
populations (cognitively impaired, chemically sedated, addicted, pregnant
and/or culturally diverse).

The
exam is designed to test pain management knowledge above the staff nurse level,
but below that of advanced practice nurses and/or nurses who work in pain
clinics or centers.
Eligibility
criteria are:
-
Hold a diploma, associate, baccalaureate or higher degree in nursing.
-
Hold a current active RN license in one of the 50 states or its territories
-
Practice the equivalent of 2 years full-time as an RN in the US or its
territories
- Practiced
in a nursing role which involves aspects of pain management (e.g. pain
assessment and management, pain management education, pain research, etc.) at
least 2,000 hours in the three years prior to applying to take the exam; and
-
Have 30 hours of continuing education in the three years prior to applying to
take the examination, of which a minimum of 15 hours must be related to pain
management.
Pain
management certification review courses are currently scheduled for March 31,
2005 at the National ASPMN meeting in Albuquerque, NM. For more information go

addition, the St. Louis Regional and Greater Kansas City Chapters of ASPMN have
scheduled a review course in September at Tan-Tar-A in Lake of the Ozarks: date

Barnes-Jewish
College of Nursing and Allied Health (formerly Jewish Hospital College) also
offers an 8 week (four hours, one night a week) basic pain management course.

Oct 15, 2005. The exam is NOT only for nurses whose specialty is pain
management, but for STAFF nurses who routinely care for patients with
pain. The test questions are written to test pain knowledge that is beyond
the generic staff nurse, but NOT into the advance practice or specialty
nurse realm.
If
they read McCaffery and Pasero's book and the APS booklet, understand
it and internalize the contents...they should pass without
difficulty.
